Various downloadable forms needed for reservations and/or rentals. Customers only need to supply the forms that pertain to their reservation. Most of the forms below can be completed within our online reservation system which is accessed via the links you receive in your reservation emails.

- Each person must have their own form
- Persons under the age of 18 must have a parent or guardian sign their form (as the parent/guardian)
- Persons under the age of 18 must also sign their form as the participant
- We cannot accept modified forms
- Please contact us if you are not sure which form you should have

Please note that release forms are now available online, through your reservation link. While paper forms may be used, there will be a $2.50 fee per form implemented for processing. Please contact our office at 606-526-7238 if you need assistance with the online form, or to let us know ahead of time if a paper form will be used.

Download a credit card authorization form!
Please call to make a reservation via the phone before sending in a credit card authorization form. If possible, reservations should be paid through the invoice email you receive which has links to the online reservation system.
